COVID-19 VACCINE APPOINTMENT STATUS FOR POLK COUNTY

 

COVID-19 VACCINE APPOINTMENT INFORMATION:

This notice is taken from the Polk County, Florida, Health Department website: “The Florida Department of Health in Polk County, in partnership with Polk County Board of County Commissioners, will be closing Polk County’s COVID-19 vaccine portal on Sunday, April 18th at 11:59 p.m. The call center will remain open until Friday, April 23.  As COVID-19 vaccine becomes more widely available across the county, DOH-Polk will transition its vaccination efforts to larger walk-up vaccine point of dispensing (POD) locations across the county where appointments are not necessary.”  This notice only affects the Polk County Health Department sites.

WALK-UP EVENTS

“DOH-Polk will transition into hosting walk-up vaccine events across the county on certain days of the week. On Tuesday, April 20, DOH-Polk will be providing COVID-19 vaccines at Lake Eva Event Center in Haines City from 10 a.m. – 6 p.m., no appointment needed. Vaccines will be available to anyone 18 years of age and older as outlined in the Governor’s Executive Order.”

HOME-BOUND

Individuals who are home-bound and wish to receive COVID-19 vaccine may register on the state’s home-bound website at homeboundvaccine@em.myflorida.com. Their request will be routed to a regional incident management team who conduct home visits to administer vaccine.

APPOINTMENT SCHEDULING

Lakeland Regional Health will now be assisting DOH-Polk by scheduling anyone who wishes to receive a COVID-19 vaccine by appointment. Those interested in making a COVID-19 vaccine appointment can visit https://myLRH.org/vaccineform or call 863-904-1895 to schedule their appointment. Appointments will be available for scheduling Monday through Friday 8 a.m. to 5 p.m.

Also, local pharmacies and private providers are making the COVID-19 vaccine more widely available.  A map and list of Providers can be found HERE.
